What to Expect Next
Once a referral is received, a Case Manager will contact the individual to offer personalized support and connect them with appropriate resources. We're here to ensure no one navigates challenges alone.

Please note: If we're concerned about someone's immediate safety, we will take action to ensure they get the help they need.
