Event Forms
- Sport Club Personal Vehicle Use Form
- Clubs must fill out this form to driver personal cars to and from Travel Requests. If you are only driving yourself, you do not need to fill out this form.
- Sport Club Hosting Event Request Form
- This form must be completed 2 weeks before date of hosting.
- CWU Sport Club Practice Request Form
- This form is due quarterly to request practice space.
- CWU Sport Club Travel Request
- Must be completed 3 weeks before date of travel. Incomplete Travel Requests may be denied.
- CWU Sport Club Room and Space Reservation Request Form
- Please submit this form 4 week days before your room request date. This is to book spaces on campus like meeting rooms or tables.

- Sport clubs may submit a marketing form to get social media graphics, flyer approval, or uniform approval .
Financial Forms
- CWU Sport Club Spending Request Form
- This form must be completed to request spending of non-travel related purchases.
- Example Spending Request
- CWU Sport Club Competitive Funding Request
- This Form is to apply for extra funding. This can be submitted any time of year.
- CWU Sport Club Contract Request Form
- This form is to request a Contract for a fundraiser, sponsorship or any other services that require a contract.
- CWU Foundation Account
- Link for donations for Sport Clubs. Make sure donors write what sport and club the donation to.
- Designation Select: Other
- Example Baseball Club, Dance Club not just dance.
Budget Presentations
- These are annual presentations for sport clubs to renew their club for the upcoming year, formally ask for allocated funds, and show case their clubs events.
- The Budget Presentation consist of a Cover Letter, Budget Workbook and a presentation to a panel

- CWU Sport Club Member Expectations
- This must be completed by every athlete at the beginning of Fall Quarter. Sport club members only have to complete this form once. Members may resubmit the form if they wish to update their emergency contact information.
- Roster Form
- Clubs must submit their roster at the beginning of Fall Quarter or after quarterly try-outs. Clubs can not travel without a completed travel roster.
- Coaching Contract Agreement
- Club Coaches must complete this form annually. Student coaches do not need to submit this.
- Constitution Template
- Clubs must submit their club constitution and email a copy to sportclubs@cwu.edu in the beginning of Fall quarter.
- Student Charge Forms
- Student Charge forms are due the 4th Friday of the quarter. Student charge forms are to pay for club dues. These are non-refundable.
- New Officer Intake Form
- This form must be completed by new officers after elections. Each officer must complete the form. Officers will gain access to SURC accounting and the shared sport club email upon completion of this form.
- Sport Club End of Year Survey
- This Survey is to hear feedback on the sport club program. This is an optional survey.
