Please submit documentation as soon as possible to allow time to review your information. After an initial review, additional items may be required before we can generate a financial aid offer.
Documents submitted by the August 1st priority due date will be processed by the first day of fall quarter. Due to the volume of requests at the start of the academic year, processing paperwork submitted after August 1st will begin mid- to late-October.

26-27 Tax and Income: Parents
These forms are for parent(s) of dependent students who were selected to verify income information for the 2024 tax year.
- If there are two parents who filed taxes jointly, only one parent needs to complete the verification.
- If there are two parents who filed separately, didn’t file, or have separate assets or income, both parents will need to complete the correct form individually.
Select which situation reflects your income/tax filing status for 2024.
- 26-27 Tax Filer-Parent(s): Complete this form if the parent and/or parent’s spouse filed or will file a 2024 IRS income tax return(s).
- 26-27 Non-Tax Filer-Parent(s): Complete this form if the parent and/or parent’s spouse are not required to file a 2024 income tax return with the IRS.
- 26-27 Assets & Debts-Parent(s): Complete this form if the parent and/or parent’s spouse/partner have been selected to verify asset information. Assets should reflect the current assets as of the date FAFSA was completed.
- 26-27 Uncommon Situations-Parent(s): Complete this form if the parent and/or parent’s spouse/partner have an uncommon situation for the 2024 tax year. This includes filing extensions, amended tax returns, victims of identity theft, and non-IRS tax returns.

26-27 Verification of Identity: Statement of Educational Purpose
If you were selected for “Verification of Identity” you cannot submit your information online.
You must appear in person to verify your identity and complete a "Statement of Educational Purpose" form in the presence of a CWU employee.
- Bring an unexpired, valid, government-issued photo ID (driver's license, passport, etc.) to complete this process.
- You may complete this verification at CWU Ellensburg campus or a CWU University Center.
- If you are an online student out of WA State, you may complete this form with an authorized notary. Contact financialaid@cwu.edu to request the notary form.

26-27 Family Size
If you were selected to verify your family size, choose the correct form to submit. Unlike income information, the family size must reflect the current situation in your household.
- 26-27 Family Size Verification: Complete this form if you were selected to verify your family size and the marriage status has not changed.
- Dependent Students: One parent on your FAFSA must complete this form.
- Independent Students: You may complete your own form.

Aviation Revision Form
Complete this form for CWU Aviation students who need their cost of attendance adjusted to reflect the cost of flight lab fees. This form is required if requesting an increase to a Federal or Private loan to cover the additional cost. The person borrowing the funds must submit this form.

Complete this form if you would like to increase your cost of attendance (COA) for required educational expenses.

Dependent Care Allowance
You may submit this form for consideration of an increase to your cost of attendance if you have out-of-pocket care expenses for dependents residing with you due to attending classes during the academic year.

Parent PLUS Loan Increase
Parents can submit this form to increase an accepted Parent PLUS loan to cover required educational expenses. Additionally, your credit check must not be expired, and you must be approved for the requested or maximum amount on the PLUS application.

Revision Request
Submit this form to request a change in your cost of attendance (COA) or financial aid. The expense must be incurred during the selected academic year.
Be as specific as possible regarding the quarter(s) and amount(s) that you would like reviewed and adjusted.

Parent Refusal
Complete this form if you and your spouse/partner refuse to provide parental information on your dependent student's FAFSA.
By not providing this information for the student, they will be ineligible for income based federal and state grants like the Pell and WA College Grant. They will only be offered unsubsidized dependent student loans.

Student Only: Complete this form if you cannot provide parental information due to unusual circumstances. Documentation is required.

Complete this form if you, as the student, made an error on your FAFSA by adding the incorrect parent, and have since updated the application to with the correct parent.
These changes are not automatically approved. You may be required to revert the FAFSA to the original parent and resubmit it for processing.

25-26 Student Marital Status Update
If you have married or divorced since completing the 2025-26 FAFSA, and would like to have your aid eligibility re-evaluated, contact the Office of Financial Aid and Scholarships to discuss your circumstances.
Any appeal to update a student's marital status must be completed by December 31, 2025.
