How to Report a Hazardous Condition or Near Miss

Please do not wait for someone else to report a hazard. Let someone know right away so that it can be addressed before someone is injured.

If you believe that an unsafe working condition exists in the workplace, you have the right to and are encouraged to make a report of the condition. If you believe that there is immediate danger to life or health call 911. For reporting unsafe or potentially hazardous conditions, you have a number of options:

  • Tell your supervisor or manager.
  • Submit a work request to Facilities Management by calling (509) 963-3000 or going online to the Facilities Management website and clicking on the "Submit Request" link.
  • Submit to EH&S using the Report a Hazardous Condition or Near Miss Form. 

An Unsafe Act is any act that deviates from a generally recognized safe way or specified method of doing a job and which increases the probability of an accident. It must contain an element of unsatisfactory behavior immediately before an accident that was significant in initiating the event. Examples of unsafe acts include choosing shortcuts, using defective equipment, and lack of attention.

An Unsafe Condition is an unsatisfactory physical condition existing in a workplace environment immediately before an accident that was significant in initiating the event. It is a condition where something exists that varies from a normally accepted safe condition and can result in injury, death, or property damage, if not corrected properly.

Unsafe Equipment includes any boiler, heating equipment, electrical wiring or device, flammable liquid containers, or other equipment or machinery on the premises or within the structure which is in such disrepair or condition that it is found to be a hazard to life, health, property or safety of the public or occupants of the premises or structure.

A Near Miss is defined as an unplanned event that did not result in injury, illness, or damage, but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality, or damage from occurring.

Filling out the form:

  • Report unsafe conditions and hazards promptly! Submit the form as soon as possible.
  • Complete all parts of the form clearly and completely. Be specific as to the location and the issue(s). EH&S will conduct an investigation of the site.

Washington Industrial Safety & Health Act (WISHA) regulations require that employees be provided with a method to report hazards anonymously. The hardcopy report form can be used for this purpose. However, anonymous reporting is not encouraged because we will not be able to ask questions to assist with an investigation. Additionally, we may not be able to provide a report of our findings.
