Occupational Health and Safety Program


All CWU personnel (faculty, staff, and students) named in an IACUC protocol who are using live vertebrate animals are required to enroll in CWU’s Occupational Health and Safety Program (OHSP). This entails completing a Vertebrate Animal Studies Medical Form and Medical Questionnaire. To enroll in the OHSP or renew medical clearance as required on an annual basis, please complete the form below.

Forms are to be hand delivered to Kerri Larson, ARNP at CWU Student Health Services - located on the corner of 11th Avenue and Poplar Street - or mailed to the below.

The forms will be reviewed by the medical clinic's OHSP medical professionals, who may request further information (i.e. basic medical tests or a physical). In addition, the OHSP professionals may request follow-up actions, such as vaccinations for those working with potentially infectious animals. Those planning international travel and research are advised to enroll in this program well in advance of their anticipated travel dates.

Medical clearance from the OHSP must be completed before any new IACUC protocols will be approved.

Students may not need to receive medical clearance for some protocols. See the FAQ page for the policy on training requirements for educational protocols.
