How to submit a Report for Concern and Support

The Concern and Support Form is an online reporting mechanism for all members of the CWU Community through Guardian Conduct. You can log in with your CWU credentials OR reports can be submitted anonymously by clicking the "Concern and Support Form" button below. If you want to submit a report anonymously, we encourage you to open an incognito window before clicking the button below.

We have divided up the different incident types to better direct the reports we receive. If you are unsure which incident type to choose, click "Behavioral/Student Conduct" and we will route it to the correct office for you. Our current incident types are detailed below:

Concern and Support Form

Academic Dispute Resolution

Support and conflict resolution services, such as communication coaching, mediation, and/or facilitated conversations for disputes or difficulties between faculty and students Disputes or communication difficulties between faculty and students needing mediation, conflict resolution advising, and support. This does not include academic concerns appropriate for the Academic Early Alert or instances of academic dishonesty.

Behavioral/Student Conduct

Potentially concerning behavior of students or non-students on campus. It also includes reports fo Academic Dishonesty, potential violations of the Student Conduct Code (on or off campus) or other potential university policy violations. Campus Housing Incident

Potential concerns or behaviors that take place in our campus housing facilities (Residence Halls or On Campus Apartments). This includes community concerns, roommate issues and potential violations of the Residence Hall/Apartment Policies and Expectations or the Student Conduct Code. All policies and expectations can be found on our Student Responsibilities and University Policies page.

Case Management/Student of Concern

Requests for assistance or support with mental health, physical/medical concerns, basic needs, family problems, grief/loss, general distress, navigation of systems/processes for access to resources, etc. that do not rise to the level of an immediate emergency (911) call.

Concerns about Faculty/Staff

Any concerns regarding CWU employees (student staff, professional staff, faculty, etc.) including but not limited to potential violations of employee policies or expectations.

Inclusivity or Bias Related Concerns

Receive support or report behavior, conduct, speech, images, or expression that discriminates, stereotypes, excludes, harasses, or harms anyone based upon their actual or perceived identity such as race, color, ethnicity, national origin, sex, gender identity or expression, sexual orientation, disability, age, or religion.

Sexual Misconduct/Title IX

Any potential Sexual Misconduct incident involving a member of the campus community, including but not limited to sexual harassment, sexual intimidation, sexual violence, intimate partner or domestic violence, and/or stalking. May include referrals and support, as well as formal investigation.

Lifecycle of a Behavior of Concern Report (PDF)

After a report is submitted, depending on the type of report received, staff will either work to connect individuals with appropriate resources and services on campus or will process the information through our Student Rights & Responsibilities process and will schedule a meeting with the student to discuss the situation and any potential violations of university policies.

Please note that the Office of Student Rights & Responsibilities follows the Washington State Administrative Procedures Act Chapter 34.05 RCW. All students have a right to due process.
