Concern and Support Form


The Concern and Support Form is an online reporting mechanism anyone can use to report behaviors that are concerning OR let the University know that someone in our community needs support. 

  • Available virtually 24 hours a day, 7 days a week
  • For all members of the CWU Community, as well as anyone outside of our University community
  • To report issues, concerns, or complaints about CWU students or employees, as well as CWU offices and departments or the University as whole

*If you have concerns about an imminent threat, life-threatening crisis, or other emergency, please call or text 911.

Concerns or complaints might look like potential/alleged:

  • Violation(s) of University policies or expectations;
  • Violation(s) of state or federal expectations;
  • Discrimination on the basis of protected class, including sexual violence; 
  • Criminal misconduct; and/or
  • Behaviors that negatively impact other members of our community.

Needing support might involve assistance related to:

  • Basic needs (e.g., food, housing/shelter)
  • Mental/physical/emotional health and/or distress;
  • Assistance navigating a campus process or issue;
  • Transition challenges; and/or
  • Navigating unexpected circumstances.

Concerns, complaints, or the need for support can be reported through our online reporting mechanism; however, you can also contact the appropriate area/office to report directly (in-person or via phone, email, or internal reporting mechanism) using the information below.
