Concern and Support Form

The Concern and Support Form is an online reporting mechanism anyone can use to report behaviors that are concerning OR let the University know that someone in our community needs support.

Available virtually 24 hours a day, 7 days a week
For all members of the CWU Community, as well as anyone outside of our University community
To report issues, concerns, or complaints about CWU students or employees, as well as CWU offices and departments or the University as whole

Reporting Concerns or a Needs for Support

Wildcats are a part of the solution when others need help! Use the Concern and Support form to if you have a concern/complaint or know someone is in need of support!

Concern and Support Form

Concerns or complaints might look like potential/alleged:

Violation(s) of University policies or expectations;
Violation(s) of state or federal expectations;
Discrimination on the basis of protected class, including sexual violence;
Criminal misconduct; and/or
Behaviors that negatively impact other members of our community.

Needing support might involve assistance related to:

Basic needs (e.g., food, housing/shelter)
Mental/physical/emotional health and/or distress;
Assistance navigating a campus process or issue;
Transition challenges; and/or
Navigating unexpected circumstances.

What Happens After I Submit a Concern and Support Form?
After a Concern and Support form is received, a team of trained University staff members will review the information you've provided and redirect the report to the correct area for response.
- Unless you are reporting anonymously, we will let you know that we have received the information you've shared with us, ask any follow up questions we might have, and let you know what the next steps will be.
Will the Information I Submit Be Confidential?

CWU keeps all information submitted is kept as private as possible; however, the Concern and Support form is not a confidential reporting form.
Do Employees Have to Report Certain Information?
Most CWU employees have reporting responsibilities related to hazing and sexual harassment or misconduct and the Concern and Support form can be used to fulfill these reporting requirements.

Reporting or Talking to Someone Directly

Concerns, complaints, or the need for support can be reported through our online reporting mechanism; however, you can also contact the appropriate area/office to report directly (in-person or via phone, email, or internal reporting mechanism) using the information below.

Internal Audit Hotline and Reporting Form

Internal Audit conducts a variety of audit and advisory related audit engagements designed to proactively identify risks and to evaluate internal controls, with the ultimate goal to strengthen operations of Central Washington University.

Internal Audit also works as a consultant in areas of cash handling, conflict of interest, compliance and operational internal controls.

Clery Campus Security Authority (CSA) Reporting

CSAs report allegations of crimes (attempted or completed) that are made in good faith meaning there is a reasonable basis for believing that the information is not simply rumor or hearsay. CSAs are mandated to immediately report allegations of crime to law enforcement when brought to their attention. CSAs must not further investigate on their own after reporting. Incidents can be reported to CSAs by anyone: a third party, witness or victim.

Reporting Crimes or Community Safety Concerns

The mission of the University Police and Public Safety is to work in partnership with our communities to create a safe and secure environment through education and enforcement. Criminal activity and/or community safety concerns should be reported to the CWU Police Department (CWUPD).

The University Police provides police services to the Ellensburg, WA campus of CWU. University Police operates 24 hours per day, 7 days per week, providing commissioned and state certified officers with complete arrest and investigatory authority.

Reporting Accessibility Barriers or Disability Discrimination

The Americans with Disabilities Act of 1990 (ADA) is a federal statute which provides protection to individuals with disabilities in the areas of employment, public accommodations, state and local government services, and telecommunications. Similar protections are provided by Section 504 of the Rehabilitation Act of 1973.

Central Washington University (CWU) policy, the ADA, and Section 504, prohibit discrimination on the basis of a person’s status as a person with a disability, require equal opportunity and access, a process for a person with a disability to request a reasonable accommodation, and a grievance process for an individual to complain of discrimination.

Report Ethics Concerns

Central Washington University (CWU) is committed to providing a learning, working, and living environment that promotes personal integrity, civility and mutual respect. As all University employees are expected to be familiar and comply with state ethics laws, rules, and other related policies, this webpage provides an overview of expectations around and resources related to ethics in the workplace at CWU.
