Occupational Health and Safety Program
All CWU personnel (faculty, staff, and students) named in an IACUC protocol who are using live vertebrate animals are required to enroll in CWU’s Occupational Health and Safety Program (OHSP). To enroll in the OHSP or renew medical clearance as required on an annual basis, please complete the form below.

Medical clearance from the OHSP must be completed before any new IACUC protocols will be approved.
The forms will be reviewed by the medical clinic's OHSP medical professionals, who may request further information (i.e. basic medical tests or a physical). In addition, the OHSP professionals may request follow-up actions, such as vaccinations for those working with potentially infectious animals. Those planning international travel and research are advised to enroll in this program well in advance of their anticipated travel dates.
