Forms and Documents
Psychology Major and Minor Applications:
Please select the Psychology Major/Minor Application form if you wish to apply for the Ellensburg-based Psychology major (45 or 60 credit majors) OR for the Ellensburg-based Psychology minor (30 credits).
You may also complete the downloadable version of the Psychology Major Application or minor application Psychology Minor Application and email it to psychology@cwu.edu or deliver it to the Psychology Department office (Psychology Building, room 421).
Affirmation of Service or Research Assistant Experience:
Students must complete a minimum of 10 hours of volunteer experience in civic engagement and/or as a research assistant for a Psychology Department faculty member or graduate student. The 10-hour requirement must be met sometime after the student is accepted into the psychology major and before completion of PSY 489. The Psychology department is using this form for this requirement: Affirmation of Civil Engagement or Research Form.

The Graduate School provides a number of documents for all of our constituents: future and current graduate students, CWU faculty, and alumni. Visit the Graduate Studies and Research webpage for more information. Prospective students should use the on-line application to apply for all graduate programs in the Psychology Department.
