Concern and Support Form
The Concern and Support Form is an online reporting mechanism anyone can use to report behaviors that are concerning OR let the University know that someone in our community needs support.
- Available virtually 24 hours a day, 7 days a week
- For all members of the CWU Community, as well as those outside of our community
- To report issues or concerns about CWU students or employees
*If you have concerns about an imminent threat, life-threatening crisis, or other emergency, please call or text 911.
Reporting Concerns or a Needs for Support
Wildcats are a part of the solution when others need help! Use the Concern and Support form to if you have a concern or know someone is in need of support!
Concern and Support FormConcerns might look like:
- Potential violation(s) of University policies or expectations;
- Discrimination on the basis of protected class, including sexual violence;
- Criminal misconduct; and/or
- Behaviors that negatively impact other members of our community.
Needing support might involve assistance related to:
- Basic needs (e.g., food, housing/shelter)
- Mental/physical/emotional health and/or distress;
- Assistance navigating a campus process or issue;
- Transition challenges; and/or
- Navigating unexpected circumstances.

What Happens After I Submit a Concern and Support Form?
After a Concern and Support form is received, a team of trained University staff members will review the information you've provided.
- Unless you are reporting anonymously, we will let you know that we have received the information you've shared with us, ask any follow up questions we might have, and let you know what the next steps will be.

Will the Information I Submit Be Confidential?
CWU keeps all information submitted is kept as private as possible; however, the Concern and Support form is not a confidential reporting form.

Do Employees Have to Report Certain Information?
Most CWU employees have reporting responsibilities related to hazing and sexual harassment or misconduct and the Concern and Support form can be used to fulfill these reporting requirements.
