Report an accident

Life-Threatening Emergency:

Call 911

FIRST, ensure treatment for the injured person

  • Call 911 for serious injuries and life-threatening emergencies.
  • For non-serious but severe injuries or incidents, go to the nearest medical facility

Accident Reporting Form

Immediately Report to Environmental Health and Safety, by calling 509-963-2255, for any of the following:

  • Death
  • Hospitalization
  • Amputation
  • Loss of an eye
  • Any other major injury

Non-Local: For all employees and students outside the Ellensburg main campus, please go to the nearest Urgent Care Center. If the nearest facility is closed or if the injury is serious: Go to the Nearest Hospital

Reporting Instructions

For any occupational injury, illness, first-aid, or near-miss incident:

First aid only:

  1. Ensure the injured employee receives prompt medical attention
  2. Complete the Accident Report Form on the EHS Webpage. Answer all questions. Use a separate form for each injured employee.
  3. Supervisors should complete an incident investigation to determine causes and corrective actions that will prevent re-occurrence.

Injury beyond first aid:

  1. Ensure the injured employee receives prompt medical attention
  2. Notify EHS and HR
  3. Complete the Accident Report Form on the EHS Webpage. Answer all questions. Use a separate form for each injured employee.
  4. Notify medical personnel that the injury occurred while at work (this will start the process for workers' compensation).
  5. Ensure completion of the Report of Accident (Workplace Injury, Accident, or Occupational Disease. (Filled-out by the employee)
  6. Supervisors should complete an incident investigation to determine causes and corrective actions that will prevent re-occurrence.
  7. If the investigation is difficult or complex, EHS is available to assist. Call 509-963-2255, or email ehs@cwu.edu. The incident investigation should be done as soon as possible after the incident.

VEHICLE ACCIDENTS

Vehicle accidents that occur off University property must be reported to police agency having jurisdiction (City, County, or State). This applies to University vehicles and private vehicles on University business. Vehicle accidents on University property must be reported to University Police. Any accident involving a University owned vehicle, no matter how slight the damage, must be described on a Vehicle Accident Report form (S.F.137) within 24 hours through the CWU Motor Pool Office, located in Facilities Management.

Non-Employee Incidents

For incidents involving visitors or students, (i.e.: not a CWU employee) a University Supervisor, or other University staff, should complete the “Student/Visitor Incident Report form”. Do not use this form for employee incidents. Please follow the instructions provided on the form.
