Refund Reissue Request

Only complete this form to request that CWU cancel your refund check if you have not received it after two (2) weeks from origination. Upon submitting this form, CWU will review your account and verify the check is still outstanding. You will receive an email informing you that we have put a stop payment on the original refund check and a new check has been issued. If you are a parent of a CWU student waiting for a PLUS loan refund, please list your child’s CWU ID number below. Checks that have been issued for less than two weeks will not be canceled. Please note, checks are not available for pickup and we encourage signing up for direct deposit.
