Only complete this form to request that CWU cancel your refund check if you have not received it after two (2) weeks from origination. Upon submitting this form, CWU will review your account and verify the check is still outstanding. You will receive an email informing you that we have put a stop payment on the original refund check and a new check has been issued. If you are a parent of a CWU student waiting for a PLUS loan refund, please list your child’s CWU ID number below. Checks that have been issued for less than two weeks will not be canceled. Please note, checks are not available for pickup and we encourage signing up for direct deposit.

I agree that by submitting this form, I acknowledge that I am requesting CWU Student Financial Services to cancel and place a stop payment on the original refund check issued to me over two (2) weeks ago. In the event I should receive the canceled check after submitting this form, I agree to VOID and return the check to CWU Student Financial Services office immediately, even if I have not received the new check being issued to me. I understand that any attempt to deposit or cash the original canceled check, will be considered fraudulent and result in fines, fees and loss of privileges. If I inadvertently cash the original canceled check I will return the funds immediately to CWU Student Financial Services, and pay the fines and fees associated. I understand if there is any current balance on my account, the refund may need to pay the balance before any refund is reissued, depending on the credit being refunded.
