Security Incident Report
This form will send an email to CWU's Security Services Team. We will review the incident report and engage other functional areas of the institution, as needed. Below are some categories with examples of why you may want to contact us:
General Information Security Issue
- You have questions on general information security topics
- You suspect your password has been compromised
- You suspect there is malware on your system
- You are aware of a network intrusion or other information security incident
Sensitive Data Issue
- You suspect there has been a data breach with unauthorized access to protected data
- A removable device with protected data on it is missing
Compliance Incident
- You are aware of a security incident involving credit card information, Protected Health Information (PHI), student records, or student personal data
Investigation Request
- You suspect that a device was used in an information security incident or it was used inappropriately
Report a Lost or Stolen Device
- You are aware of a lost or stolen device
