COTS-SURE Application Form

Be sure you are familiar with all of the eligibility requirements and conditions of the COTS-SURE program before you start the application process.

Application instructions can be downloaded here.

You will be asked to upload several documents prior to submitting your application. This includes your and your faculty mentor's contact information, your preparatory research experience, how your proposal fits into your academic and professional goals, a detailed project description (including hypotheses, research design and methodology, and literature citations) as well as a detailed budget.

Please provide a list of your preparatory coursework and/or experiences. If you are already trained to carry out the project, it should be indicated; otherwise, describe the form of mentoring needed that will occur.

PROJECT DESCRIPTION

Please include the following:

1. Title of your project

2. Goals and Objectives - include a clear statement of the hypotheses being tested or the research problem being explored. (Approximately half a page)

3. Background and Significance (1-1.5 pages)

4. Research Design and Methods - include a timeframe for project completion. (1-2 pages)

5. Literature Cited - must include fully cited, peer-reviewed sources. (May be single-spaced)
