General Information

PLEASE NOTE: Since the building will be locked down for the first two weeks of winter quarter, there will be a curbside pick up for instruments on the first four days (Jan. 5-8) You still need to do the checkout using this form, but you will need to come to the double doors by the band room and text me when you're here to pick up your instrument(s). Personal Lockers

Due to social distancing requirements, instrument and locker checkouts will primarily be done by submitting the form below. You will only receive a locker appropriately sized for your primary instrument. You can not get a large locker until all our student's have lockers for their primary instrument. If you truly believe you need multiple lockers, feel free to state your case in the comments section, but this is no guarantee you will receive one at this time.

Instruments

The form below also serves as a means to request an instrument, whether for methods, ensemble use or your primary area of study. You may request as many instruments as necessary, but please limit your requests to only those instruments which are absolutely necessary for your course work. Unfortunately, you will need to submit a separate request for each instrument, however, if you are requesting instruments for a methods class which uses two instruments, like high brass for example, you only need select the appropriate methods class from the bottom of the list and you will receive both contracts.

Contract Processing

In order to complete the check out, after submitting this form, you will receive a notification via text message informing you of an e-mail I sent. You must to e-sign the attached PDF contract(s) and return them to me before you will receive a locker number and combination, again via text. I will process requests as quickly as possible, in the order in which they were received. The hours in which requests will be processed is limited to my normal office hours of M-F 7:00 - 12:00, 1:00 - 4:00.

Please complete the form carefully, especially your student ID and primary instrument selection. If your student ID does not match my records, or if you request a bass locker for your piccolo, your checkout will be delayed.
