Prospective Graduate Students:

 Prospective students should use the on-line application. If a hard copy application is preferred, please contact the Graduate School to provide the form.

Letter of Recommendation Form
 

 Recommendations are a part of the on-line application. If you prefer to use another means of supplying recommendation letters you can obtain a recommendation form on the Apply to the CWU Graduate School page.

Application Fee Waiver
 

 Use the Fee Waiver form if you think you are eligible to have your admissions application fee waived based on your financial situation. For details and the form please go to the Apply to the CWU Graduate School web page.

Course of Study Form Degree Programs

 List the courses that you will take toward your master's degree. You will need to file this form before you have accumulated 25 quarter credits towards your degree and may be required to submit this form earlier if you are applying for grants, fellowships, or financial aid.

Course of Study Form Certificate Programs

 Students who are enrolled in Graduate Certificate Programs should submit this form to the Graduate School shortly after admission into the certificate program.

Course Substitution Form  download PDF | DOC

 You will need to file this form if you need to replace a class on your course of study (degree or certificate earning programs) with an alternative class.

Graduate Committee and Option Approval Form

 Lists your graduate committee and the option (thesis, examination, or project) that you are selecting for your degree. File with the Graduate School as soon as you have formed a committee and selected your thesis option.  It may also be required for certain CWU grants and fellowships.

Human Subjects Review Committee (HSRC)             Link to the website

 Any research at CWU that involves human participants must receive review and approval by CWU's human subjects and administrators. 

Institutional Animal Care and Use Committee (IACUC)  Link to the website

 Any research at CWU that involves non-human animals must receive review and approval by CWU's animal protections administrators.

Thesis General Regulations Download

 Thesis General Regulations should be followed by all students completing the thesis option.

 This form will be filled out by students completing the thesis option.

Final Folder Check

This is your REQUEST to Graduate or Earn a Graduate Certificate

All graduate students must complete the on-line folder check request for a final review of their file no later than the first week of their final quarter.

Results of the request and applicable forms are emailed to your CWU Email Address!

If for any reason you do not graduate or earn your graduate certificate the quarter that you have requested the folder check for you must complete another folder check for the quarter that you will finish.

Name, Gender, or Social Security Number Change Form
 

Notification of changes to your your name, social security number, or gender must be made through the CWU Registrar. Refer to the website link below and scroll down to Additional Forms and choose Name, Gender, or Social Security Number Change

Graduate Student Research Support

 

 Semi-Annual awards designed to help students with supplies, equipment or travel expenses related to their research for their thesis or project.
 

Master's Student Summer Fellowship
 

Summer fellowship stipends of $3,500 designed to help students focus on research for their thesis or project 

Student Travel Grant 

 Travel funding is available for graduate students who are presenting research or creative work.

 This document allows the Graduate School to better coordinate with you on your grant proposal.

Faculty Research Support Award
 

Monetary support for research or creative activity up to $3,000.  
Faculty Travel Award

Funding is available for faculty who are presenting research or creative work is available through the Office of the Provost per CBA Article 18.12.2.  Please find the application form on the Office of the Provost website under Faculty Relations then forms/Applications:  Tenured/Tenure-Track Travel Fund Guidelines and Application
