The PEBB Program offers life insurance through MetLife (Plan number 164995-1-G). This is a summary of benefits only. To see the certificate of coverage, go to:

As part of your benefits package, you receive — at no cost to you — $35,000 of life insurance coverage and $5,000 of accidental death and dismemberment (AD&D) coverage. You have the option to buy additional coverage by enrolling in the supplemental plans offered.

Optional Life and AD&D Insurance available through MetLife.

Additional coverage may be purchased:

  • For Employees: Increments of $10,000 up to $500,000 with no Medical Evidence of Insurability, to a maximum of $1,000,000 with Medical Evidence of Insurability.

  • For Spouse or State-Registered Domestic Partner: If you are enrolled in Optional Life Insurance, you may apply for amounts of Optional Life Insurance for your spouse or state-registered domestic partner in increments of $5,000 (up to one-half the amount of Employee Optional Life Insurance you obtain for yourself).

  • For Children: If you enroll in Optional Life Insurance, you may apply for Child coverage in $5,000 increments up to $20,000.

If you see “Enter your company name,” start typing “PEBB” (without the quotes) and a dropdown list should appear. Select “PEBB Benefits – State of Washington.”

Life insurance enrollment must occur within 31 days of eligibility. After the initial  period, proof of good health to add life insurance must be provided.
