Employee COVID-19 Vaccination Verification
COVID-19 vaccinations are required of all CWU faculty, staff, and students.
Vaccination Verification
Employees can provide their proof of vaccination by:
- Visiting the HR office in Mitchell Hall First Floor Monday-Friday 8 a.m. - 5 p.m.
- Scheduling an appointment via Bookings to virtually verify vaccination record
Vaccinated employees must present the following:
- Vaccination Card
- Or documentation from a health care provider
- Or documentation from a state immunization information system record showing that the individual is fully vaccinated
Originals, copies, or photographs on a mobile device are acceptable.
If you've misplaced your card, visit MyIRMobile, to access verified proof of your COVID-19 vaccination.
- Employee ID Number
- Employees can locate ID number on CWU Connection Card or in MyCWU.
If verifying online, only an original or copy is acceptable – viewing a picture on a phone screen virtually is not sufficient for verification.
- Medical Accommodation Request

Instructions for employee:
- Complete employee section of form above and send to medical provider to complete provider portion.
- Email completed form to ada@cwu.edu, drop off at HR, First Floor Mitchell Hall, or fax to (509) 963-1733.
- After the form is submitted, an HR team member will determine eligibility for accommodation. Accommodations will be determined through an interactive process including the employee, supervisor, and appointing authority.
The Associate Vice President of HR will review and make the final determination on requests for accommodation, where questions exist concerning undue hardship or reasonableness of the requested accommodation.
