CWU students seeking a medical or religious exemption from any vaccination requirement must fill out the Vaccination Exemption Form.
If, in the opinion of a medical provider, the following required immunization(s) would be injurious to the health and well-being of the student, a medical exemption may be granted. A student with a medical exemption may be excluded from the university during a disease outbreak. The length of time a student is excluded from the university will vary depending on the type of disease and the circumstances surrounding the outbreak and could range from several days to over a month. A Certificate of Immunization Exemption for medical reasons is valid only when signed by a licensed physician (MD), doctor of osteopathy (DO), nurse practitioner (NP), or physician assistant (PA). Examples of medical exemptions, may include:
- Autoimmune disorders
- History of severe, or life-threatening allergies (anaphylaxis)
A religious exemption may be granted to a student only if the immunization(s) conflicts with a genuine and sincere religious belief. Information about the specific documentation needed for exemptions will follow in the coming weeks. A student granted a religious exemption may be excluded from the university during a disease outbreak. The length of time a student is excluded from the university will vary depending on the type of disease and the circumstances surrounding the outbreak and could range from several days to over a month.
What if I don't comply?
Students who arrive on campus inadequately immunized, or without an accepted exemption form, will be sanctioned per university policy and a hold may be placed on your registration until the requirement has been met.
Step 1: Register
Visit Med+Proctor and click "register" to start a new account. Type in your school email address to get started and follow the directions to register a new account. This service is free for all CWU students. You are not required to pay for, or use, any additional services that may be offered.
Step 2: Download
Fill out any required personal, medical or insurance information. Download the required forms and follow the directions provided.
Step 3: Upload
Log back in to your Med+Proctor account and upload a copy of your forms. Make sure your forms are complete and legible. You will receive an email confirmation once the forms have been reviewed.
