• Am I eligible to apply for student health insurance?
During the open enrollment period, all registered undergraduate students taking 6 or more credit hours, graduate students taking 3 or more credit hours, post-graduate students taking 1 or more credit hours, pre-doctoral interns, visiting scholars, research scholars, and international students are eligible to enroll in the Student Health Insurance Plan. All students must actively attend classes for at least the first 31 days after the date for which coverage is purchased. Home study, correspondence, and television (TV) courses do not fulfill the eligibility requirements that the student actively attend classes. The Company maintains its right to investigate student status and attendance records to verify that the policy eligibility requirements have been met. If the Company discovers the eligibility requirements have not been met, its only obligation is to refund premium. Eligible students, who enroll, may also enroll their dependents. Eligible dependents are the spouse and dependent children. Dependent eligibility expires concurrently with that of the insured student.
In addition, all registered graduate assistants with active assistantships taking credit hours that have elected the student health coverage, by completing the Student Health Insurance Acceptance Form will be automatically enrolled for the quarters which they are performing their assistantships. Registered graduate assistants who are not granted a summer assistantship will not be automatically enrolled in the student health plan by the University for the summer quarter. Graduate assistants may enroll directly with the company at www.4studenthealth.com/cwu for the summer quarter, provided that the graduate assistant was enrolled during the spring quarter prior to the summer quarter.
