Citation Appeal Form

1. Parking Citation may be appealed WITHIN 15 DAYS OF DATE OF ISSUANCE
2. The appeal process suspends payment of the monetary penalty until a decision is rendered by the Appeal Board.
3. ONE CITATION ONLY PER APPEAL FORM
4. You will receive an e-mail or mailed letter providing the appeal results normally within 10 days. It is your responsiblility to verify your appeal results. Please contact the CWU Parking Office if you do not receive the decision.

By submitting this appeal form you are electronically signing this form and agreeing that all information is true and correct. You will be sent an e-mail after the appeal board meeting informing you of the appeal board decision. Please refer to the Appeal Process page to view the process if you have any questions. If there are errors after hitting the submit button simply fix the errors and hit submit again.
