DE Incident Reports

The Distance Education Incident Report online form provides a means to track problems that may occur at the CWU onsite and offsite distance learning classrooms. Instructors, operators and technical staff can file the incident reports. When someone fills out the request an e-mail is sent to all those on the Distance Learning E-Mail list to notify everyone. Video Conference Request

Faculty and staff can schedule video conferences between any rooms on campus, at the Centers, at other institutions or businesses with video conferencing capabilities. To carry out a request please fill out the online form at the following website:

Video & Multimedia Services Request

Faculty and staff can fill out this online form to request videotape duplication in a variety of formats, including source to DVD. For convenience in regard to your video and multimedia needs, please use the URL below. Media Equipment Service Request

Any faculty or staff having difficulty with media equipment on campus can use this form to report failures and malfunctions. Media engineering staff will be notified by e-mail and follow through in replacing or repairing the faulty equipment.
