in residence halls and on campus.

Binder and inside cover page.
2) Table of Contents Be sure to include all
headings and page numbers.
3)Introduction--describe the organization, its mission
statement , and your project (1-2 pages)
4) Learning goals and objectives (2-3) goals and 6-10 learning objectives: What
did you set out to learn? How did you learn it? Write them in the
Objective-Activity-Evaluation (OAE) method described in
class and in your packet. These must be clearly stated (1-3 pages).
Consider your learning in all Three Domains of Learning and your improvement in
at least three of the Seven Competencies of an Entry Level Health Educator.
5) Volunteer Experience Description (4+ pages)
b) Fully describe your duties and how you performed them. One page per person.
c) Give a day to day personal report and accounting of your time (2-3 pages per
person). This may be titled, Journal
or Daily Log. Each daily log will be
reviewed and verified by each memberÕs signature.
6) Experiential Learning In this section, describe what your group learned
about your organization or school, what you learned about yourselves and how
you feel youÕve improved as health education professionals.
a) Refer to the Seven Competencies of a Health Educator (packet & website).
In an item-by-item style, explain how your group acted in each of the Seven
Competencies that applied to your volunteerism.
b) In an item-by-item style, refer back to each learning objective and describe
how your group achieved or failed to achieve each objective. (3-5 pages)
7) Letter of Thanks Use the formal business letter format as in your proposal
letter. Thank your site supervisor for allowing you this opportunity to
advance your knowledge and your career. Mail them a signed copy and
provide me a signed copy in your volunteer report notebook.
8) Appendices. Use this section to include photographs, flyers,
handouts or lesson plans pertinent to your volunteerism. Each appendix
should have a title page preceding it. (Example: Appendix A, Lesson Plans followed by a clean copy of the lesson
plan.)

SEVEN COMPETENCIES of a
CERTIFIED HEALTH EDUCATION SPECIALIST
The following seven competencies have been identified and established through
the joint efforts of the Association for the Advancement of Health Education
(AAHE), the National Task Force on the Preparation and Practice of Health
Educators, and the National Commission for Health Education Credentialing, Inc.
1) Assessing individual and community needs for Health Education.
2) Planning effective Health Education programs.
3) Implementing Health Education programs.
4) Evaluating effectiveness of Health Education programs.
5) Coordinating provision of Health Education services.
6) Acting as a resource person in Health Education.
7) Communicating health and Health Education needs, concerns, and
resources.

Create three new flyers for the ACRP using Pagemaker. Time 3 hours

Attend all pertinent staff meeting relating to the
ACRP. Time 3 one hour sessions.

of 7: 00 am and 3:00 PM on 10/30
Assist the nursing staff at the ACRP between the hours

Attend review session following the ACRP. Date
