The University provides $25,000 of life insurance and $5,000 of accidental
death and dismemberment insurance to eligible employees at no charge.

| Part C Optional, - Between 50% and 100% of your annual
salary (see footnote) is automatically approved if applied for within the
first 60 days of eligibility. Coverage increases automatically with salary increases.
Later enrollments Require a medical questionnaire and approval. |
Part B, Basic Dependents- $2,500 for spouse and unmarried dependent
child aged 14 days to 24 years. (You may add this coverage for new spouses and/or
dependents within 60 days of eligibility by completing a new enrollment form.) |
| Part D Supplemental, - Up to $50,000 is automatically
approved if applied for within the first 60 days of eligibility. Later enrollments, or
amounts over $50,000 and up to $350,000 require a medical questionnaire and approval. |
Part B, Supplemental Spouse- (Must have chosen Part B Basic above to
apply for this coverage.) Up to 1/2 of employees C & D combined. Up to $25,000 is
automatically approved if applied for within the first 60 days of eligibility. Later
enrollments, or amounts over $25,000 require a medical questionnaire and approval. |
| Part E, Accidental Death or Dismemberment -
From $25,000 to $250,000 is available for employees. Dependents may also be insured at
a reduced amount of coverage. |
