Business English: Form Template
Modifying A Form Template to Make Your Own
It is very good to use the cut and paste method to create a new form. Once you understand the basic elements of a form, you can modify an existing form and put in your own information.
Below is a form your teacher made for you to use as a template. You can highlight the kind of form tag you want and paste it into another position on the web page. Then, replace the pieces of information you want with your own questions. Usually this means replacing the name and value attributes and also the part of the code that will show up on the page, for example, your own questions.
You can use the handout about how to view your form on Netscape as you are making your changes to make sure it is working correctly.
