We will send a Key Reissuance Authorization notice to the department.
The department determines who should be reissued keys and who should
not. If there are persons who should not receive the new key, cross
the individual's name off the report. The report needs to be signed
by the department head, chair or dean. Return the original signed
Key Reissuance Authorization report to the Lock Shop. The lock shop
will not complete the rekey work until it receives the Key Reissuance
Authorization report. In cases where adequate time has passed and
the Key Shop has not received the report, the lock will be rekeyed
and anyone needing access to the room will be asked to bring a NEW
key card signed by the department head, chair, or dean.
