Alternative Testing Service Form
As a service to faculty, the Center for Disability Services (CDS) offers the Alternative Testing Service (ATS). Professors who do not wish to personally provide testing accommodations for their students with documented disabilities may arrange for their exams to be administered with accommodations (i.e., extended time, quiet/private testing room, reader, scribe, computer, adaptive equipment, etc.) by completing this instruction sheet and returning it, along with a schedule of testing dates, to the Center for Disability Services.
If you would like CDS to assist you by providing alternative testing services to your students with disabilities, please complete this form and return it to Bouillon Hall, Room 205, MS 7431, or FAX it to 963-3235 prior to the first exam for this course.
