The WIN-WIN Request Form page requires the use of JavaScript. Please

For each required section, we have requested detailed information that WE NEED in order to quickly and efficiently process your replacements under the WIN-WIN program.
Because of this need, you will be unable to submit the form without completing the required sections. The page is designed so that you will NOT need to repeatedly enter duplicate information (Thank you Jesse Days and Miho!). The webpage design will automatically fill in information for each section if it can be carried forward.
For clarity on the form, some fields will not be changeable. We have tried to make the form easy to read and understand so you can see what will take place based on your choices.
Once you submit this form, you will receive a copy of the email that we use to generate our internal work orders.
This WIN-WIN program form can only be used to replace computers within individual departments. This program is not designed for cross-department transfers.
We request a contact name for communication and coordination between our department and yours. This is the person to whom we will issue the charge/credit and property transfer forms. End users will be contacted directly to set up delivery appointments unless specific instructions are sent in the Comments box provided on the form.
Please provide all of the Budget information requested so that the entire process can operate smoothly and efficiently with no delays once begun.
You may choose to replace your old computer with either a CWU Standard PC, OR a CWU Standard Mac Mini.

You can request one of three final actions for the computer being replaced:
- Send the replaced computer directly to surplus.
- "Trickle" this computer to another location within your department.
- Do Nothing. (This is a new choice added with this revision!) We will transfer any data off of the replaced computer, but then we will set it off to one side and do nothing further with the unit. The WIN-WIN program requires that you provide us with a surplus computer, but it does NOT have to be one we are replacing.

A "Trickle" computer is a machine currently in your department which we will move to a location of your choice (under this program, only ONE "Trickle" will be permitted). We will only perform the trickle on the old machine being replaced under this program. If you need other machines moved around in your department, please contact the Helpdesk at 963-2001, or email them at helpdesk@cwu.edu to request these moves (Soon, with the implementation of "Employee Self-Service", you will be able to request regular ITS services directly from your browser!). WIN-WIN is a special replacement program, and we are coordinating many transfers with limited resources. Thank you for your understanding in this matter.
The two choices available for the "Trickled" computer are:
- Send this unit back to CSS as the Surplus unit.
- Do Nothing (please see above for details).
This is the computer that will be leaving your department. In almost every case, these old computers will be permanently removed from the CWU campus. The spirit of this program is to raise the general level of technological capability for the entire campus by removing older, slower (obsolete) equipment.
IF you have not selected "Surplus" in any of the above sections, you can now specify the location of the computer we are to pick up. In some cases, the computer a department wanted to use as the "Surplus" unit was not in the same location as the other systems.
Please use this section for any special instructions that you need to pass along, or for suggestions on how we might improve our design and/or clarity of this site to provide you with better service in the future. It is our intention to make this website easy to use and understand, streamlining the processes involved in replacing the older technology on campus with newer, faster, more powerful equipment.
