Science Honors Research Program : Application

The Application Form with instructions can be downloaded either as a Word document or as a PDF document.

The complete Application must contain:

1. CWU Science Honors Research Proposal Cover Sheet
2. Science Honors Research Proposal Abstract Form
3. Research Proposal Project Description including
   1. Title of project,
   2. Goals and objectives,
   3. Background and Significance,
   4. Research Design and Methods,
   5. Dissemination Plan, and
   6. Literature cited.
4. Completed Budget Description:
   1. Budget Cover Sheet, and
   2. Budget Details and Justification
5. Animal/Human subjects clearance (only if applicable).
6. Endorsement of the student by the faculty mentor; attach a letter.
7. CV of faculty mentor.
8. Statement of career goals, a list of completed science and math coursework, GPA, and a list of extracurricular activities.
9. Co-Mentor form, if more than one faculty will be sharing mentoring responsibilities.

Submit the original application with six copies to the COTS Dean's office (Bouillon 203) by 4:30 pm Friday, January 18. An electronic version of parts 1-4 of the application (in PDF format) must also be emailed to the director (piacsek@cwu.edu).
