Publicity Request Form (PDF)

This form is for any group or department that would like to request marketing or graphic design services from the Publicity Center. Please return this form to the Publicity Center and arrange an appointment with one of our staff to help make your event a success. Please note, a "Budget Number" and an "Authorization Signature" must be filled out on the form before any work on your project begins.
