Forms are available at academic department offices, Registrar Services and University Center offices. Permission must be granted from the instructor, department chair, and dean or associate dean prior to registration. To complete the form, make sure to include the descriptive title and course outline. Once the appropriate signatures have been obtained, submit the individual study permit to the Registrar Services to complete the registration process. The form must be completed prior to registration and submitted prior to the end of the Add/Drop period.
