    * Your voluntary service experience (s) through the Don and Verna Duncan Civic Engagement Center.

    * A 500 word essay describing your leadership experience in community service, student clubs, organizations, or other activities, and how your experiences have developed your leadership skills
    * Two letters of recommendation from faculty, community leaders, and/or program advisors

    * A letter addressing how you meet the requirements and why you want to major in Construction Management
    * Two letters of recommendation, at least one or which from someone in the construction or engineering industry

    * A one page essay describing your educational/career goals, your honors/awards, your involvement in civic or extracurricular activities, and reasons why you are the best candidate for the scholarship
    * Three (3) letters of recommendation from non-relatives who can speak to your character, academic ability, and potential for success
    * A one page resume
    * Unofficial transcripts from any college or university you attended before CWU

    * A one page essay describing your educational/career goals, your honors/awards, your involvement in civic or extracurricular activities, and reasons why your are the best candidate for the scholarship

    * A portfolio of 6 to 10 pieces of original artwork in two or more media
    * A statement of interest and goals in art (not to exceed 250 words)
    * One letter of recommendation from a high school art instructor
    * Optional: A self-addressed stamped envelope for return of portfolio

    * An essay describing why you believe you are now capable of quality performance and achievement
    * Provide two letters of recommendation: one academic and one non-academic

    * An essay of 2 pages or less addressing your career goals in the Safety & Health Management industry, involvement or membership in professional organizations, and university and/or community volunteer experiences.
    * Two letters of recommendation -- one must be from a faculty member in the Safety & Health Management program.  Preference given to students who submit a third letter of recommendation from a community leader (e.g. church, city, or county official, etc.).

    * Explain career objectives. Be specific as to the types of occupation, teaching emphasis, and activities toward which you would like to orient your career
    * Describe the physical education courses you have completed to date, and how they have benefited you in terms of your career goals
    * Explain your involvement in the field of physical education through participation in professional conferences and/or workshops, voluntary or paid positions in teaching, community service, or other related experiences
    * Explain why you should be chosen as the recipient for this scholarship. Include details of which personal interests, talent, or personality traits make you an exceptional candidate

    * Three (3) letters of recommendation from people who can attest to applicant's academic progress and his/her involvement in outside activities as listed above.

*A 1000-1500 word essay addressing the following: Your motivation for entering your chosen career field, your long-term goals, and how you expect to impact your chosen career field above and beyond what others might achieve.

*A 500 word essay detailing your rodeo and community service/leadership activities-include length of time involved, level of involvement, names of organizations, associations/memberships, and awards earned

*One page essay describing your involvement in leadership through extracurricular activities and/or your involvement with people with special needs

*Submit a one page essay on your career plans and why journalism is important

*Two (2) letters of recommendation from CWU faculty members

*Two letters of recommendation from faculty attesting to his/her potential in pursuing a History major

A one page essay expressing why you deserve a "second chance" at CWU

A statement from our high school counselor certifying that you have completed a college prep curriculum, including the courses described above. DO NOT SEND A TRANSCRIPT. Failure to provide a completed statement from your school counselor outlining your college prep curriculum will result in the disqualification of your application.

Additional materials must be postmarked or submitted to the Scholarship Office on or before March 1st.

An essay of 2 pages addressing your career goals in the Safety and Health Management profession involvement or membership in SHM related professional organizations.
Two letters of recommendation- one must be from a faculty member in the Safety and Health Management program.

One essay (2 pages maximum, doubled spaced, 12 pt font) demonstrating:

    * Applicant's academic interest in Philosophy or Religious Studies
    * Applicant's contribution to the department through non course related, discipline related activities
    * How Philosophy or Religious Studies will be incorporated into the applicant's future career or life path

Submit a 2-page essay describing a humorous event or experience in which you have been involved

Submit only one set of Additional Materials if you are applying for more than one Education scholarship:

    * Three letters of recommendation: Two from CWU faculty in two different academic disciplines and one from a previous college advisor qualified to evaluate your character and potential for becoming an excellent teacher

Two letters of recommendation from former faculty/teachers addressing the following:

   1. A challenge faced by the student
   2. How the student overcame the challenge
   3. A description of the student's academic improvement
   4. The student's potential as a leader

•     Submit a personal statement in which you describe your intellectual interests, achievements, and future aspirations.  Include a description of how these interests have been demonstrated through participation in departmental activities, or other campus activities, or community service.

•     Provide a letter of support from a faculty member in your academic department.

• A double-spaced written statement containing the following: A clear, concise account of your reasons for seeking this scholarship and a description of your interest in physics. (Entering Freshmen: 300-word limit; Entering Transfers: 500-word limit)

• Letters of recommendation from the department chair and a department faculty member.
