Patient Portal Registration

Welcome to Central,

Your health and the health of the campus community is important.  With so many students attending Central it’s essential for us to be vigilant of health related hazards.  Being properly immunized is one of these health concerns.  Within the first month of starting classes all incoming students should complete their immunization/insurance information on the Medical and Counseling clinic patient portal.  If you have questions about immunizations please call: (509) 963-1881. 

Completing this objective will clear the reminder from your MYCWU page.  Please allow up to 30 days for the task to be updated.

Please follow these steps to set up your confidential access on the Student Medical & Counseling Clinic (SMaCC) patient portal.

 

REGISTRATION ON THE PATIENT PORTAL

 

BEFORE STARTING:  (Important Information!) You will need your CWU Student ID, CWU e-mail address (not your CWU Email Login), immunization information, and your medical insurance card.

(Please note: You will need to have a scanned, saved copy of your Immunizations and  the front and back or your Insurance cards in order to Upload the information)

3.Sign Up for an account. Click on Register or click on the highlighted link to access the sign up page.

You’ll need the following information;
1. Create a user name 
2. Your CWU Student Identification
3. Your CWU E-Mail address-Not Your CWU E-Mail Login. (This can be located on your MyCWU page)

***Once this step is completed you will receive an email with a link to  instruct you on creating a password.

ENTERING AND UPLOADING IMMUNIZATION AND INSURANCE INFORMATION

IMMUNIZATION: Complete this task by entering your immunization information. Click Immunization drop down and Enter Dates.

 

 

5.

INSURANCE: Complete this task by entering your insurance information. Please be sure to include any letter prefixes or suffixes, group number, and the date of birth of the policy holder. 

 

6.

UPLOAD DOCUMENTS: Once you have input the data information you will need to upload a copy of your insurance card (front and back), and an official copy of your immunization record. Click on Upload.


 

7.Congratulations! This is one less task to complete. This task will be removed from your MyCWU manually by the SMaCC Staff. (***May take up to 30 days for changes to reflect in MyCWU.)

Steps to take if you receive this Message:


 

 

1. Check that you are entering the correct CWU Email Address. ex. first.lastname@cwu.edu or lastnamefirstinitial@cwu.edu (also known as your Network ID).

2. Are you entering your Student ID correctly?

3. Have you had your name changed? ex. Jon vs Jonathan

4. If you are still unable to Register contact Student Medical & Counseling Clinic 509-963-1881.
