Employee COVID-19 Vaccination Verification

COVID-19 vaccinations are required of all CWU faculty, staff, and students.

Vaccination Verification

Employees can provide their proof of vaccination by:

  • Visiting the HR office in Mitchell Hall First Floor Monday-Friday 7 a.m. - 5 p.m.
  • Scheduling an appointment via Bookings to virtually verify vaccination record

Vaccinated employees must present the following:

  1. Vaccination Card
  • Or documentation from a health care provider
  • Or documentation from a state immunization information system record showing that the individual is fully vaccinated

Originals, copies, or photographs on a mobile device are acceptable.

If you've misplaced your card, visit MyIRMobile, to access verified proof of your COVID-19 vaccination.

  1. Employee ID Number
  • Employees can locate ID number on CWU Connection Card or in MyCWU.

If verifying online, only an original or copy is acceptable – viewing a picture on a phone screen virtually is not sufficient for verification.
