SURCHARGES BEGIN JULY 1, 2014

If you are currently enrolled in medical insurance and do not indicate through “My Account” on the PEBB Health Care Authority website by May 15, 2014, that you do not use tobacco and that your spouse/partner does not have other coverage, you will pay premium surcharges starting July 1, 2014.

Tobacco Surcharge: $25 per month due as of July 1, 2014
A monthly $25 surcharge will be assessed in addition to your insurance premium if you or any enrolled family member uses a tobacco product.

To avoid surcharge: If you and all enrolled family members have been tobacco free for two months, you must indicate on “My Account” on the PEBB Health Care Authority website www.hca.wa.gov/pebb

If you do not respond, you will pay the monthly $25 surcharge effective July 1, 2014.

Spouse/Partner Surcharge: of $50 per month due as of July 1, 2014
A monthly $50 surcharge will be assessed in addition to your insurance premium if you enroll a spouse/partner, and that person has chosen not to enroll in his or her employer’s medical coverage that is comparable to our coverage.

To avoid surcharge: If you have a spouse/partner enrolled on your medical insurance who does not have comparable coverage, you must indicate on “My Account” on the PEBB Health Care Authority website: www.hca.wa.gov/pebb

If you have a spouse/partner enrolled on your medical insurance and do not respond, you will pay the monthly $50 surcharge effective July 1, 2014.

Wellness Incentive: for 2015
Receive a $125 discount off your medical deductible in 2015 (if not enrolled in a CDHP) OR Receive an additional $125 in your HSA in 2015 (if enrolled in a consumer-directed health plan)

To receive the incentive: You must indicate on “My Account” on the PEBB Health Care Authority website: www.hca.wa.gov/pebb that you have:
1. Chosen a primary care provider, and
2. Completed your medical plan’s health assessment (Group Health or Uniform Medical), and
3. Started or participate in a wellness activity
