Appropriate Documentation

Academic adjustments/accommodations are determined on an individual basis by Disability Services (DS) professional staff and are based on disability documentation and an interactive interview with the student or employee. Individuals with disabilities requesting services through DS are required to provide documentation from a qualified professional that:

  • Verifies the existence of the disabling condition;
  • Identifies the functional limitations caused by the disability;
  • Describes the affect the disability has on the individual’s ability to access the educational or employment process.
