Continuing Education Course Proposal Form

Continuing Education is pleased to assist you with devleoping a successful course. The completion and thoroghness of this form is the first step in assuring quality programming.

Course proposals are accepted on an ongoing basis. Continuing Education staff will use the information provided to assist in scheduling non-credit courses.  If your course is accepted, a Program Manager will contact you to coordinate details and gather additional information.

Please note: All CE programming requires a course evaluation form from each particpant. You as the instructor will be responsible for the distribution and collection of the CE Evaluation Form at the end of your course. To view the Course Evaluation Form, please click here.

Please provide the amount of compensation you would like to receive for instruction of this course. This can be an hourly rate or a lump sum payment. This will be used to develop a budget for the course and can be negotiated or changed as needed.
