Immunization Checklists
Immunization Verifications: Print out the verification form as required by your specific major department. Make an appointment with the Student Health Center at 509-963-1881, or with your family doctor. The Student Health Center appointment is free and will last approximately 20 minutes. The purpose of the appointment is to verify your immunization records. After verification is obtained at the Student Health Center or your doctor's office, bring the signed form to Career Services along with copies of your immunization records.
