1 . Department secretaries or appropriate center staff members will distribute student evaluation packets to each faculty member during the ninth week of instruction (third week of the 4 1/2 week summer sessions). The packets will include Form A--Lecture Discussion evaluation forms unless Forms B, C, or D are requested by the faculty member.
2. Follow the "Guidelines for Administering the Student Evaluation of Instruction (SEOI)" provided by the Testing Services Office.
3. The Testing Office produces the summary evaluation from the preprinted forms, and returns the forms and written comment sheets directly to the department early the following quarter.
4. Department secretaries produce copies of all student comments. Copies of the summary evaluation and the student comments are distributed to the department chair, dean, and faculty member. The dean's copy will be placed in the faculty member's Professional Record File.
