CC Form 136-R - Briefing on Government Sponsored Benefits for ROTC Cadets
CC Form 137-R - Government Access to Student Records
CC Form 139-R - Cadet Enrollment Record
DA Form 3425-R - Medical Fitness Statement (This can be signed by a family physician family with your medical history. If you do not have a physician that is familiar with your medical history see the instructions below for Sports Physical)
CWU Sports Physical Form - This can be used in the event of a cadet not having a family physician who can sign the DA Form 3425-R above. Students must bring this form to the CWU Health Clinic on campus and make an appointment for a sports physical.
Citizenship Documentation - This form states what documents can be brought in for proof of citizenship.
