regarding our insurance policies, Athletic Medical Insurance: Covers injuries to Athletes participating in high and low risk sports in CWU's Athletic Program. Call Ken Kladnick at (509) 963-3238, Head Athletic Trainer, for coverage limits and claim forms. Boiler & Machinery: Sudden and accidental damage, including breakdown of covered objects or part thereof that requires repair or replacement. Please contact Insurance Program Manager for Limits of Coverage. There is a $5,000 deductible. Commercial Property Insurance: The current premium rate is .117 per $100 of value. The current property damage deductible is $250,000 per building location per occurrence. Coverage is for the revenue generating buildings, i.e., Residential Services and the SUB. Catastrophic deductible for earthquake and flood is 3% of insured values at any one location. Commercial Property Insurance: Electronic Data Processing (EDP): The policy covers risk of direct physical loss, including Earthquake and Flood, to data processing equipment whether you own, rent or lease it from others, or it is otherwise under your control. EDP must be inventoried and values reported through the Inventory Control Sheets provided to each department by the Property Management Office for yearly inventories. Please contact Insurance Program Manager for limits. Deductible is $250,000 per location per occurrence.
General Liability Self-Insurance Policy: The State's Self-Insurance Fund is used to pay tort claim judgments and settlements arising from general liability and vehicle accidents for which the State is found to be negligent. All forms are available at the State OFM Office of Risk Management website. Please contact the Insurance Program Manager with all claims information and questions. Graduate Student Health Insurance Plan: Major/Medical injury and sickness insurance plan for all registered Graduate Teaching Assistants taking credit hours at CWU and working as a Graduate Teaching Assistant. Graduate Studies and Research automatically enroll and pay the premiums for Plan I - Standard Plan. Visit the MEGA Life and Health Insurance Company website at http://www.studentinsurance.net/public/enrollment/SelectSchool.aspx?type for brochures, enrollment cards (printable using Adobe Acrobat), coverage receipts, ID cards, claims status and other services. Master Automobile Policy: All state owned vehicles are insured for liability under the self-insurance fund but NOT for physical damage. Departments purchasing a vehicle for departmental use are responsible for their property insurance premium if the vehicle meets one of the four following criteria:
If the vehicle meets one of these criteria, then specific drivers are assigned and Drivers Questionnaires are completed for each driver. Drivers must have 7 years licensed driving experience. Please contact the Insurance Program Manager for information. Master Day Care Policy: Special Risk Pre-Schoolers Accident Insurance policy for registered students attending the University's licensed Day Care program. Please contact Insurance Program Manager for information. Public Employees Blanket Bond (also known as Fidelity Bond): Public Employees Blanket Bond, including faithful performance of duty. Loss sustained by the Insured through any fraudulent or dishonest act or acts committed by any of the employees, acting alone or in collusion with others, during the bond period; subject to policy exclusions and limitations. Discovery period is one year. Public Officials Bond: Pecuniary loss of money or other personal property sustained by and/or the responsibility of the insured resulting through failure of official duty by employees occupying scheduled positions. International Travel Insurance: This plan takes over in a foreign country, where your regular hospitalizations and medical policies may not cover. This policy provides accident and sickness, emergency medical evacuations, and repatriation of remains coverage while traveling abroad. Please contact Insurance Program Manager regarding coverage. Aviation Liability and Aircraft Insurance: Please contact Insurance Program Manager before acquiring or first using aircraft on behalf of CWU. Insurance coverage is NOT automatic. Coverage for this exposure is specifically excluded from our state self-insurance policy. Foreign
Liability Insurance: The Foreign Liability Policy provides
general and automobile liability coverage for state employees traveling on
state business, outside the United States. The policy provides
worldwide coverage excluding the USA and its territories, Puerto Rico,
Cuba, North Korea, Libya, and Iraq. The University's self-insurance
liability extends only to exposure in the U.S. and Canada. No other
foreign liability insurance is provided. Marine Liability and Hull Insurance: Please contact Insurance Program Manager before acquiring or first using watercraft on behalf of CWU. Insurance coverage is NOT automatic. Coverage for this exposure is specifically excluded from our state self-insurance policy. Property Insurance: Except for Computer Equipment and property obtained through the sale of bonds, such as Auxiliary Services Housing and Dining facilities, the University does NOT insure against property loss. Likewise, the personal property of employees is not covered unless by their own homeowner's or renter's policy. Rented Vehicles and Equipment: The University does not carry property insurance for rented vehicles or equipment, with the exception of vehicles rented on the state contract with Enterprise Car Rental. Therefore, anyone renting a vehicle or equipment on behalf of the university from any rental agency must purchase additional coverage of physical damage waiver. ***At all times, university employees should try to rent vehicles from Enterprise under the State of Washington contract #05104. This contract provides coverage if the rental vehicle is damaged as well as liability coverage.*** Student Exchange Visitor Plan: International students or scholars with J-Visas taking credit hours, registered for thesis or dissertation or otherwise engaged in educational activities outside their home country or country of regular domicile at institutions of higher learning in the United States are required to have an insurance plan that meets USIA guidelines. Students who have applied for permanent residency are not eligible for the J-1 plan. Please contact Insurance Program Manager for information. Student Field Trip Insurance: Student Field Trip Insurance is a secondary insurance for those who have medical insurance. It is primary insurance for those who do not have medical insurance. The annual deposit has been paid. There is no charge. Professors or organizers of academic field trips must list all participants of the field trip on the Field Trip Insurance Form. Student Health Insurance: Medical injury and sickness insurance plan for students who have no other medical insurance. Brochures may be obtained at Business Services and Contracts, the Cashiers Office and the Student Health and Counseling Center. This is an additional plan that can be purchased by students who are enrolled in the student health insurance plan. Student
Professional Liability Insurance (Medical Malpractice):
This program insures the professional liability of students who furnish
mental and physical health-related professional services required under
curriculum or internships. Examples include athletic trainer, mental
health counselor, school counselor, dance therapist, drug and alcohol
counselor, marriage and family counselors, audiologist, language-speech
pathologist, dietitian, physical therapist, social worker, occupational
therapist, and services in the physical education, health and recreation
fields. Any academic department within these and similar fields should
make the Student Medical Malpractice Insurance available to students. The
limit of liability is $1,000,000 per occurrence with a $3,000,000 annual
aggregate. The cost is $13.18 for coverage through the duration of the
annual master policy period, beginning and ending on September 1st. It is
the student's responsibility to make sure their premium is paid and their
coverage is current. Coverage for this exposure is specifically excluded
from our state self-insurance policy. Download the form at StudentMedicalMalpracticeInsurance.doc Summer Camps: There is no pre-arranged coverage for this exposure. University camp directors should include coverage for their participants by making arrangements for a fee to be added to each participant's registration. Coverage includes limited medical insurance for each participant. Please contact the Insurance Program Manager for information.
